Please read and respond to this short form about your Covid 19 symptoms or Exposure.
Practically Perfect Children's Services LLC is keeping track of all of our providers and their Covid 19 status. It is extremely important that we do everything possible to avoid sending a provider to a job when there is potential for exposure or infection.
You have been asked to complete this form because you advised us of an exposure incident or are having symptoms that could indicate a Covid 19 infection or have recently tested positive for the Covid 19 virus.
IMPORTANT NOTES:
1. If you believe you were exposed to the Covid 19 virus but you have no symptoms, its extremely important that you wait 5 days before preforming a Covid 19 test since it can take up to 5 days after exposure to show up on a test.
2. If you currently have symptoms that may signal an infection (sore throat, cough, fever, chills, shortness of breath, body or muscle aches, headache, loss of taste or smell, congestion or runny nose, nausea or vomiting, or diarrhea), you should immediately take a Covid 19 test.
3. The day of your exposure, positive test, or first symptom is is considered day zero. Keep a day count from that point forward. In most cases a person is able to return to the workplace (with masking) on day 6 if they do not have symptoms. If you still have symptoms on day 6 you may work 1 day after the first day you experience no symptoms.
4. If you tested negative and you have no symptoms, you may return to work immediately. If you tested negative but do have symptoms, you may work on the day after your first day with no symptoms.
